This year the school is doing things a little different.

To help expedite the physical process they have provided all the forms online

You can fill out and submit the Athletics and Activities Consent and Signature page, and the OSSAA Eligibility Form (Athletes only) electronically.

Create a login and fill out the forms and print the physical form to have ready when you take your student for their physical. This website also gives the option to track your student’s athletic forms to ensure they are good to go.
